The Team
Susan L. Conley, APRN
Susan is a Board Certified Family Nurse Practitioner who graduated with honors from Chamberlain University. She has 26 years of experience in healthcare and manages our team to ensure all our backgrounds work together to help support your health goals.
Susan is a Florida native, raised in the Glades area, and worked at Lakeside Medical Center for 15 years prior to obtaining her nurse practitioner licensure. Susan has broad nursing experience ranging from pediatrics, medical-surgical, telemetry, womens health, and geriatrics.
Additionally, Susan spent two years training and working under a physician who specialized in geriatrics, internal medicine, and nephrology. Susan is also a clinical adjunct professor at Palm Beach State College who frequents PCA, LPN, and RN students in the clinical setting.
About Us
Florida Elder Care & Wellness is a home-based mobile medical service for the elderly population. Our focus is to provide medical care to the older patients in their home who have difficulty going in office to see their primary care physician. Our practitioner can take the role of PCP or as a geriatric consultant to assist the patient's PCP as an added layer of health care.
Our services are primarily covered under Medicare with very small or no out of pocket fees. We provide primary care services, Alzheimer's/dementia care, chronic care management, and assess for geriatric syndromes such as fall risk and frailty, weight loss, urinary incontinence, insomnia, memory loss, and more.
We also assist in nursing home or assisted living placement. On occassion the patient may need hospice or palliative care. At Florida Elder Care & Wellness, we can help make the determination and provide support to families in difficult times of decision making.
